Text - puts space for the user to write a short answer. The spreadsheet will simply list all responses.
Paragraph text - same as Text, but with more space allowed.
Multiple choice - You create a list of items and each has a Radio Button in front of it. The user must choose just one. You have the choice of adding Other which is a text box. the user chooses it and types in their response.
Checkboxes - The list has checkboxes in front of them. The user can click on as many as they wish.
Choose from a list - the User clicks on an arrow which then shows a list of items. They choose just one from the list.
Advanced
Scale - You choose the beginning and Ending number and label each of them. The User is directed to choose a number within that scale in response to your question.
Grid - Create a grid of buttons in rows and columns. The user can mark one button per row.
Date - shows calender where respondent can choose a date
Time - response must be a time
Layout
Section header - Puts text between sections allowing you to give the user directions or simply help organize a long survey.
Page break - Breaks the form up into "pages." The user sees a Continue button at the bottom of the page, until they get to the last page where the button says "Submit."
Image - allows you to place a picture in the form. You can use a URL, take a picture if using a device that allows it, or use one you've previously saved to a file.

User Options

In the top left corner, you may select from three options:

Allow users to edit responses - If you turn this on, the user can come back to a form and change the answers put in earlier. If they do so, the information on the spreadsheet will change. It will not add another line for that user.
Require P-H-Mail sign-in to view this form - The user has to have a P-H-M Email account and be signed in to it to view the form. If they aren't already signed in, the sign-in box will appear when they try to open the form.
Automatically collect respondent's P-H-Mail username - Turning this on makes the user's name show on your spreadsheet that collects the answers. The user will see a note at the beginning of the survey warning that their name will be collected.

NOTE: You will automatically collect the date and time the form was completed on your spreadsheet regardless of your choices.
